Completed supplementary form

Passport valid for at least 6 months beyond exit date

2 passport-size colour photos *** taken on a white background *** within last 6 mnths

Copy of flight confirmation

Photocopy of passport information page and previous Zimbabwe visas

For business visas, all the above plus a company letter and invitation both on headed notepaper. The company letter should include the applicant name, purpose of journey and organisation to be visited plus an acceptance of financial responsibilty for the applicant. The invitation should include the applicant name and purpose of journey.
